Mail In Order Form
Use this form if you will be mailing or faxing in your order. Be sure to fill in all fields as a blank field could cause a delay in fulfilling the order.
New Account Form
Set up a departmental account with GT Distributors.
Federal Excise Tax Exemption Certificate
If your department is exempt from Federal Taxes you must complete this form. A form is required for each manufacturer whos products are ordered. This form is currently the only one authorized by B.A.T.F. Fax or mail the form along with your departmental order of Firearms or Ammunition. Your State Tax exemption can not be used for Federal Tax exemption purposes.

This form is partially filled out with the proper information for ATK Products Federal Cartridge, CCI, and Force on Force. If your department is exempt from Federal Taxes you must complete this form. A form is required for each manufacturer whos products are ordered. This form is currently the only one authorized by B.A.T.F. Fax or mail the form along with your departmental order of Firearms or Ammunition. Your State Tax exemption can not be used for Federal Tax exemption purposes.

The forms listed below are needed for tax exempt orders from the states listed.
GT Quote Request Form
If you would like a quotation for a large volume order, you can email, fax or call us with your request. Please allow 2 business days for processing.
Badge / Name Bar Order Forms
- Allow 6-8 weeks for delivery because each badge is custom made.
- Put only one badge style for each order form. If anything differs (lettering,
size, color, style number) use separate forms.
- State Seal - be sure to fill in the name of your state on the line beside
"STATE SEAL". Also, be sure to check whether your seal should be
(1) Plain (2) Plain with ENAMEL RIM or (3) Full Color (Enamel is Standard)
- Fill out form completely and carefully. Be sure ALL necessary blanks
are filled in and boxes checked.
- Payment must accompany order, on individual orders.

This is the badge order form that must accompany every Blackinton badge order.

Use this if you are sending in your name bar order with a Mail in order form or are sending a PO with multiple names.

Firearm Purchase Authorization Letter
Sample letter authorizing purchase of firearms. This letter must be retyped on department letterhead and hand signed by the authorized officer and supervisor. No copied letterheads will be accepted.

Letter of Intent (must be printed on Departmental letter head)
This form is needed with each departmental order of Automatic/Restricted firearms and/or some destraction devices from Defense Technologies.
Ammunition Authorization Form
ALL ammunition orders must have a signed copy of our Ammunition Authorization Form faxed/mailed in with/before the order is placed.
Body Armor
Safariland Universal Armor Fit Forms
The new Safariland Universal Fit forms are split between NIJ-04 and NIJ-06 standards. Please insure that the form you are filling out has the vest you are sizing listed in the "General Information" before filling out the form. NOTE: You MUST wear your duty rig during sizing. If you do not have your duty rig with you, we will not be able to properly fit your armor.
